Eden Prairie School District Reserve/Substitute Teacher Requirements

Please visit the Eden Prairie School District Human Resources Department and complete the information required to become a Reserve/Substitute Teacher.

We require the following information for all Reserve/Substitute Teachers:

• Complete Reserve/Substitute Teacher Application
  
• Valid MN teaching license (All short-call substitute applicants will follow the licensure procedure established by the Minnesota Department of Education, http://education.state.mn.us/html/mde_home.htm)
  
• Pre-Employment Criminal Background Check (Completed by Eden Prairie School District)
  1. $15.00 payment made to Eden Prairie School District.
  
  
• Right To Know Training (Training regarding district policies)
  
• Employment Packet
  1. Employment Eligibility Verification Form (I - 9)
  2. W - 4 Form
  3. 2 forms of Identification
  4. Direct Deposit Form (voided check is needed)
  5. Personal Information Form
  6. EEO Form
